First Steps After School Registration Information
1. There is a once a school year registration fee of $25.00 for each child. 2. Total After School cost is $45.00 per child per week. 3. After School fee must be paid on or before the 1st of the month. 4. No child will be allowed to participate if fees are not paid on time! 5. There will be an additional fee for field trips, depending on transportation and admission fees. 6. No child will be admitted into camp before 3:00pm and there will be a late fee of $2.00 every 10min. for each child after 7:00 pm. 7. Each child must be sign into camp by an authorized adult and no child will be allowed to leave until an authorize adult checks them out. 8. After School program starts the first day of the students school year and end the last day of school, Monday-Friday 3:00 to 7:00 pm. 9. The After School Program is open to all youth grades K - 12 (both genders). 10. Sparkle, reserves the right to refuse or dismiss anyone who does not abide by the rules and regulations. 11. Summer Camp will begin the first Monday after the last day of school and run for 10 weeks. Registration fee $50.00 Camp cost 90.00 per week, per child. 12. First Steps Curriculum In today's Society knowledge of how to operate computers is a necessity. No matter the age, everyone needs to have a high level of computer literacy. We offer an after school program that focus on the academics and elevate the level of computer base skills. Home work help is always available and given first consideration. The program operates from 3:00-7:00pm Monday – Friday during the school year. The summer camp focus on students increasing their knowledge of computers and advance their minds in academics, performing arts, field trip, visual arts, craft activities, library programs and other fun and creative learning activities are all a part of our exciting ten week summer camp program. Don’t let your child’s mind go without gaining knowledge after school or during the summer time, register them now. As the saying goes, “a mind is a terrible thing to waste”. The camp opens at 7:15AM and closes at 6:00PM, program activities operates from 8:00 AM to 5:00PM.
Parent After School Agreement Sheet

I understand that I must give a two week notice and must pay for those two weeks, if I decide to take my child out of the program or if my child is dismissed from the program (A panel discussion before dismissal). I fully understand and have read the requirements for my child to participate in the program. Parents preferred pick-up time between 5:30 – 7:00pm, must pick-up their child or children by 7:00pm. There will be a late fee charge of $2 for every 10 minute after 7:00 pm.

Release Form
In consideration of Sparkle Theatre Productions, Inc.(STP) grant of an audition and/or selection to participation or assist in a performance staged by STP First Steps Summer Camp Program, I hereby waive any and all claims for myself and my heirs and for any child or ward in my care of custody so participating (the “Minor” named below) against the STP First Steps Summer Camp Program and any of its agents, officers, and employees, for injury or illness which may directly or indirectly result from my participation, and I further agree to forever hold said parties harmless and agree to indemnify each of said persons or property undersigned (if parents or guardian may arise by virtue of the undersigned engaging in the activities associated with the STP First Steps Summer Camp Program. I further state that I am (and if for a minor, the minor is) in proper physical condition to participate with the STP First Steps Summer Camp Program. It is agreed that any participant may be photographed and video taped at the direction of the STP First Steps Summer Camp Program and agree that Sparkle in its sole discretion may use such photograph’s and videotape’s. Furthermore, the undersigned (if parents or guardian of the Minor named below) do hereby expressly stipulate and agree to indemnify and forever holds harmless the STP First Steps Summer Camp Program, its successors and assigns, employees, representatives, agents, officers, and directors, against loose from any and all further claims, demands, or actions in law or equity that may hereinafter at any time be made or brought by the said Minor or by anyone on behalf of said Minor for the purpose of enforcing a claim against the STP First Steps Summer Camp Program for damages on account of any injuries stained in consequence of participation in an camp activity by the Minor.
